Our Care Process

Value-based Care

As with most things in life, medicine is constantly changing. At AdvocateMD we wish to continue to evolve with one common goal, better patient care.

The Current State of Healthcare

High Volume, Poor Outcomes

Having one physician managing all of your healthcare care feels like something from the past. AdvocateMD is determined to change this. When our providers can focus more on patient care rather than volume, we are able to improve outcomes and widen the scope of practice and treatments for various diseases. Unfortunately, under most current health care models, physicians are often dictated by their employers to focus on sheer volume. “I feel like I’m just a number” is something doctors hate to hear from their patients, but understand it is often the reality.

Why It Matters

The Dangers of Fee for Service Models

This production-based approach has been proven to lead to unnecessary testing, unnecessary referrals, and limited access for acute care of patients - just to name a few. This ultimately leads to poor patient outcomes and decreased patient satisfaction. Under the fee for service models, physicians get reimbursed for the volume of services provided. At AdvocateMD our goal is to provide you with the healthcare you need and deserve while recentering your care to where it needs to be - with your primary doctor.

Value-based Care

The AdvocateMD Healthcare Model & Promise

At AdvocateMD, we limit the number of patients our providers see, allowing us to have more time for our established patients. This ensures we can provide the quality and inclusive care we wish to deliver and that our patients expect from us. With a team of excellent nursing staff, clinical care coordinators, and management, as well as close relationships with other integral members of your health care team, AdvocateMD is certain we can provide you with an excellent experience in and out of our office, all while focusing on our number one goal - keeping YOU healthy.

Benefits

View the Benefits of Value-based Care

In a Value-based Care model, patients are the focus of the healthcare process. There will be coordination of care among a multitude of care teams managed by AdvocateMD. This may include home health, nursing, dedicated patient care coordinators, transportation assistance, diagnostic testing, and of course, when needed, physician specialists. Patients feel more like they are a part of the team rather than just a number.

As technology continues to improve our lives, why shouldn't it also improve your health? AdvocateMD uses advanced healthcare analytic software to identify health risks specific to you. Real-time data is used along with powerful predictive software to identify at risk patients before a negative health outcome would occur. AdvocateMD can then take steps to provide preventative care for those risks and make treatment more efficient and available. Our primary goal is to prevent disease and to keep you healthy!

In a traditional fee for service care model, healthcare providers don’t always have the ability or incentive to work with other providers to provide effective care for their patients. In a value based healthcare system, there are incentives and improved technologies that enable providers to communicate with each other to coordinate care. The integration produced by collaborative care creates better outcomes for patients because all of their healthcare providers work together.

The long-term mission of value based healthcare at AdvocateMD is to help make society as a whole healthier. The ongoing focus on illness prevention combined with accessible and efficient acute illness management will markedly improve patient’s health and improve overall wellness on a large scale.

In a value-based system, patients, healthcare providers, and insurance companies all benefit. When done well, Value-based Care lowers costs across the board. Every member of your healthcare team - including the patient, physician, insurers, and hospitals all see a reduction in expenditures. This translates to decreased healthcare spending as a whole. Why does this matter to you as a patient? When insurance companies have lower costs, they are less likely to raise premiums and deductibles, saving you money as well!
